This form is used to reuqest STEM OPT Extension for students in OPT after graduating from M.A. I/O Program; students advanced 
from M.A. I/O to Ph.D. BP or OL program and in the regular 12-month OPT after graduating from Ph.D. BP or OL. In order to be eligible 
to request STEM OPT Extension, you must be in a regular 12-month OPT and the earliest you can apply with USCIS is 90days prior to your 
OPT end date. Attending the STEM OPT workshop is mandatory before you request for the extension.

STEM OPT Validation Report Form
This is for the mandatory Validation Report students in STEM OPT must send to the DSO within 10days prior to the due date, 
even when there are no changes. (required)

Explanation of a direct relationship between OPT Employment and Major Area of Study For F-1 Non-Immigration Students to determine compliance 
with the United States Federal Regulations. Your DSO will review and SEVP will monitor and review it through your SEVIS Portal Account. ​

To update employer contact information while on OPT (required whenever changes occur with OPT employer)

This form is used to request an employment authorization document for your OPT or other Off-Campus Employment.

Only use this form to review notes from TCSPP to assist you when you are completing the official I-765 form. DO NOT submit this to USCIS.

This worksheet is for STEM OPT application, including some special notes to bring to your attention when you enter information.

This form must be completed by students and their employers when apply for STEM OPT. Student will complete the Section 1 and 2. 
Section 3 to Section 6 should be completed by Employer and Student together. The form should be signed off by the employer. 
The last section for Evaluation on Student Progress can be left blank for initial application.
