If you have tested positive for COVID-19, please notify TCSPP by submitting this online form as soon as possible. In addition, please follow the CDC Guidelines for those who have tested positive for COVID-19.
After reporting a positive COVID-19 case, you will be instructed by TCSPP's COVID-19 Support Team on what you need to do before you can return to a TCSPP campust.
HOW TO SUBMIT AN EXEMPTION REQUEST FROM TCSPP'S COVID-19 VACCINATION POLICY
Follow the instructions below to request an exemption to TCSPP's COVID-19 vaccination requirement if you are eligible to do so per the COVID-19 Vaccination Requirement Policy.
TCSPP requires 15 business days to process an exemption request. You are responsible for submitting your exemption request early enough to ensure that you are authorized to access campus or the desired event. If you receive a request for more information on your exemption submission, you must reply to TCSPP within five business days.
Medical Exemption Request
To request a medical exemption, follow the steps below:
1. Have your licensed medical provider complete and return to you the COVID-19 Vaccination Requirement Medical Exemption Request for Students form. This form must be completed and signed by your licensed medical provider.
2. Once your licensed medical provider has returned a completed form to you, submit the form online via the COVID-19 Vaccination Requirement Exemption Request portal. Remember to attach your completed and signed medical form.
Religious Exemption Request
To request a religious exemption, answer the following questions on the COVID-19 Vaccination Requirement Exemption Request portal:
- What is the religious reason for your request? Write a detailed explanation of the reason for your exemption request in your own words.
- What sincerely-held religious belief guides your objection? Write a detailed description of the sincerely-held religious belief that guides your objection.
- Does your sincerely-held religious belief constitute an objection to all immunizations or only the COVID-19 vaccination? If your belief does not constitute an objection to all immunizations, you must explain the religious basis for not receiving the COVID-19 vaccination.
