TCSPP's COVID-19 Vaccination Requirement

The Chicago School of Professional Psychology (TCSPP)​ expects all students to read our COVID-19 Vaccination Requirement policy​ and related FAQ​ which detail the COVID-19 vaccination requirements for Students and Employees at The Chicago School. All Students and Employee​s who access a TCSPP campus or attend a TCSPP-sponsored event will be required to be fully vaccinated for COVID-19, with limited exemptions as detailed in the policy.

 How to Submit Proof of COVID-19 Vaccination


Review Process
Proof of vaccination will be reviewed within one (1) week of submission. Students will only be contacted if their proof of vaccination was rejected. If you are not contacted within one (1) week, your proof of vaccination was approved.Reasons for rejection might include a blurry or unclear image, wrong image, and/or a falsified record. In the event a Student’s proof of vaccination is rejected, the Student will be informed of the reason(s) for rejection and given additional opportunities to upload acceptable proof, if necessary.  
 
After December 19, 2021, Students whose submission is rejected will be unable to access campus until such time as additional proof is submitted and accepted. Any consequences related to being unable to access campus during this time period will be the responsibility of the Student.   

Note: Student Employees are also required to submit Proof of COVID-19 via the instructions in Workday using their student employee email.

 How to Report a Positive COVID-19 Case

If you have tested positive for COVID-19, please notify TCSPP by submitting this online form as soon as possible. In addition, please follow the CDC Guidelines for those who have tested positive for COVID-19.​  

After reporting a positive COVID-19 case, you will be instructed by TCSPP's COVID-19 Support Team on what you need to do before you may return to a TCSPP campus or attend a TCSPP-sponsored event.

 How to Submit an Exemption Request from TCSPP’s COVID-19 Vaccination Policy

If you wish to request an exemption to TCSPP's COVID-19 vaccination requirement and are eligible to do so per the COVID-19 Vaccination Requirement policy, you may do so following the instructions below.

You are strongly encouraged to seek and submit your exemption request as soon as possible. Note: The deadline to request an exemption from TCSPP's COVID-19 Vaccination Requirement policy has passed. While a student may still request an exemption, the result of their request may not be available before the December 19. A student who has not received an approved exemption by December 19 may be restricted from accessing a TCSPP campus or sponsored activity.

Medical Exemption Request: To request a medical exemption, follow the steps below:
1.    Have your licensed medical provider complete and return to you the COVID-19 Vaccination Requirement Medical Exemption Request for Students form. This form must be completed and signed by your licensed medical provider.
2.    Once your licensed medical provider has returned a completed form to you, submit the form online via the COVID-19 Vaccination Requirement Exemption Request portal. Remember to attached your completed and signed medical form.

Religious Exemption Request: To request a religious exemption, follow the steps below:

  1. Write an original response to all of the following questions:
    1. What is the religious reason for your request? Write a thorough explanation of the reason for your exemption request in your own words. 
    2. What sincerely-held religious belief guides your objections? Write a detailed description of the sincerely-held religious belief that guides your objections.
    3. Does your sincerely-held religious belief constitute an objection to all immunizations or only the COVID-19 vaccination? If your belief does not constitute an objection to all immunizations, you must explain the religious basis for not receiving this particular immunization.

2.    Once you have prepared your responses to the above, sign your document and submit it online via the COVID-19 Vaccination Requirement Exemption Request portal. Remember to attach your signed written statement.​


 How to Submit a COVID-19 Test Result

TCSPP's COVID-19 Vaccination Requirement policy details when a negative COVID-19 test may be required for access to a TCSPP Campus or for attendance at a TCSPP-sponsored event.

If you are required to submit a negative COVID-19 test, please complete the and upload proof of your negative test result. Acceptable proof of a negative COVID-19 test:

  • A photo or scanned electronic copy of a negative viral test (PCR or rapid antigen)​ See CDC guidelines for more information about this type of test.
  • A medical record issued by a licensed health care provider indicating test name (must be a PCR test), date of test, and result.

If your documentation is not approved (e.g., is not legible), you will receive an email explaining the issue with instructions for how to upload the corrected information. ​
